It gets in the water, then swims, in the water. Like generally most things do when they swim. Unless they are swimming in urine, as then they probably will swim faster as it is gross.

Candiru are the little catfish that have been known to swim into a mans penis/urethra and work their way up the urethra, lodging inside. Lets just say that excruciating pain, burning, bloody discharge and obstruction to the urine flow would all be a good list of symptoms. Ouch!
The Candiru, The candiru refers to parasitic freshwater catfish in the family Trichomycteridae. They can be found in the Amazon River and are almost impossible to see through being translucent. They are eel shaped and can grow up to a size of 6 inches (15cm) in length. The fish is attracted by the smell of human urine. After tasting the urine the candiru may follow the urine stream back to the human. It then swims up the urethra and uses its spines to manifest itself somewhere in the urinary tract. After having spread the spines like an umbrella the candiru finally starts to draw blood and may grow by the amount of blood that is consumed. It eats away the muscous membranes and tissues until it is stopped either by hemorrhage or the host. Every attempt to catch the fish by the tail will fail since once in the urethra, it cannot be pulled out because the spines are spread like an umbrella. Men as well as women can be attacked by the candiru.

The candiru is tiny (5-14mm) parasitic freshwater catfish and it is small enough to get inside the human body through any of the various openings including the penis and presumably any hole of that size or larger, male or female. The candiru entering the human body has actually happened and is well documented, although they are not capable of swimming up a stream of urine as commonly thought.
No the amazon can't always be safe. E.g. There is a type of parrasite that lives in the amazon river called a Candiru that attacks when having sensed urine in the water and they attack the human or thing that released that urine
