98-01 ranger with a 4.0 liter v6
I'm not a mechanic / technician but according to what I have read the 2.5 L 4 cylinder engine used in the 1998 Ford Ranger is not listed as being an interference motor - so if that is correct your O.K. ( Helpfull - sometimes ! )

At TDC of the compression stroke for cyl #1, the cam lobes will be in a position such that they are putting no pressure on the valves, and valves are closed. At TDC of the exhaust stroke for cyl #1, the lobes will be in a position such that intake and exhaust valves may be open. Exhaust will be closing, and intake will be opening. There are two revolutions for the crank for one of the cam. TDC usually refers to TDC of the compression stroke, when valves are closed. This is when combustion/spark occurs, and is when the valves are adjusted on most engines.

I have the same problem on my 98 Ranger. I have tracked it down to a device in the heater core housing that is attached to the actuator that is adjusted by the thermostat. It apears that this device changes the vaccume signal to the hot water valve, opening and closing it. Unfortunatly, you have to remove the entire dash to open up the air plenum that this device is located in. It is not for the faint of heart.
