A receipt is always given in stores either when you exchange an item or buy a product.
Like anywhere else, if you have a recent receipt with the item on it, they should gladly exchange it. Sometimes older products do not get removed from the shelves.
Well it depends, some lets you return the item within 3 weeks of purchase and some stores only allow you to exchange the item for something else with similar value.

It depends on the store policy, but generally, any branch will accept a return or exchange of an item purchased at a different branch. This would not be true if the stores are all independently owned franchises.
Most stores allow for exchanges if the customer is not satisfied with the product. Since the picture is up to par in standards, this may be a manufacturing problem. It is not unreasonable to return the item for a refund or exchange.

Items bought from the grand exchange will either be noted, or in a box will the full set of the armor you have bought if the item is noted this is simply solved by putting the item into your bank, and withdrawing it again. if the item is in a box, you must first talk to one of the grand exchange people and exchange your box with your items in for the actual items, you then will be able to equip any item.
