University Sims can only have children if you have the mod Inteen (which allows teens and young adults to have sex and be pregnant) installed, although them having children during that stage is not a good idea because when they graduate and move back to the main neighbour hood the child is stuck at the university lot.

∙ 13y agoNo, there are no hospitals for the Sim to go to and the game is not made with any other options on how they have their baby.
Sims 3 is the only game that let's you decide, they can go to hospital or stay at home.
