You can have a misscarriage at any time. An early loss is usually caused by a chromosomal abnormality in the embryo. When it reaches the developmental stage where that genetic info is required to continue it simply cannot so it stops growing. If you have a D&C the products of conception can usually be collected and analysed. A result of normal XX could be contamination of the mom's cells who is a normal female. Sometimes there is no good explination for a loss.

Yes you can have a miscarriage throughout the pregnancy, but it's most common in the first trimester.
