If a patient chooses not to receive the treatment doctors recommend, and the patient is in the ER or in-patient, the patient will be asked to sign a "Left against medical advice" type form. It releases the doctor and facility from any liability if the patient leaves, then gets sicker or dies after refusing treatment.

Ask if you can speak to some of the residents in the day room at the facility. You should be able to get a good impression from the general physical appearance and attitude of the residents. If the residents look ill or appear to nervous about speaking to you, this could be a sign of substandard treatment. If the facility won't allow you to speak with current residents, assume the worst and mark it off your list.
If your over 18 just yourself and a local garda have to sign it. If your under 18 then yourself, one of your parents (or guardian) and a local garda sign it.
In the US, if the arraigning judge assigns a PR, Personal Recognizance, bond, then yes, in a sense, you may "sign yourself our of jail."
