In the US and Canada, dialing *67 on any landline will suppress the Caller ID display at the far end. (Note that calls to toll-free numbers and to 911 will still display your number, even if you block Caller ID.) A fax line is nothing but an ordinary landline that happens to have a fax machine connected to it. You can swap a fax machine and an ordinary telephone on the same line.
You do not have to dial a one after star 67 for out of state calls. You do need to enter the area code.

I'm not sure what you mean, can you use it more than once. But *67 blocks your phone number when you dial out. If you need to call multiple people, you will need to use *67 each time to restrict your number when you dial out. So, I guess you can interpret that to mean yes, you can use it more than once.
Star 67 (*67) is what people use prior to dialing a telephone number when they want to temporariy mask or hide the number they are calling from. *69 is what people use to call back a number if you don't have caller ID.
