I had same problem with a Ford Escort (except max heater setting was 3) and it turned out to be the fuse box! My mobile mechanic tried replacing the switch on dashboard, the ballast resistor under the bonnet to no avail. Replacement fuse box was over £200 through Ford main dealer that's without the labour cost (& that was about 4years ago) so had fusebox repaired by auto electrician at Sanderstead in Surrey at a cost of £97 Worth checking the first things I mentioned here first though.
