Washing underpants does not get them as clean as new, but will remove surface dirt and most stains. Sometimes you will need to bleach underpants or wash them several times to get them perfectly clean, especially if there are blood stains on them.
Primarily spiders, but I would figure anything small enough to carry back to their nests.
I heard baking soda and warm water works.

I've had some success with muriatic acid- although you have to be very careful & only works if stains not too deep. They sell it in pool supplies.
Oxi-clean is amazing for most stains. If it is really set in I would put resolve on it and the Oxi-Clean powder. If you don't have those products use baking soda and cream of tar tar.

If you've already tried washing it with soap and water, then try an eraser. Regular kind or the Mister Clean Eraser thingy.
