It sounds like the rear differential (In the middle of the axle) is worn out, if you are lucky it will just need minor parts and oil but better get it properly checked because if it is caught early enough you'll save on an expensive repair job.

Have your cooling system serviced. Drain, flush, and coolant replaced. Install a new thermostat.
