During the monarchy the king was the ruler. During the Roman Republic two annually elected kings were the heads of the republic and the army. During the period of rule by emperors the emperor was in charge.
Until the late 6th Century BCE they had a king. Then they had a senate and assembly of the citizens until the 40s BCE when Julius Caesar made himself Dictator for life. Then they had a line from the 20s BCE to the 3rd Century CE of 'First Citizens' who shared power with the Senate. Then they had emperors.
