In some cases, notably ear mite infestations, and conditions that cause an excess production of ear wax, cleaning may be needed, and a vet should handle that. Some breeds, such as the Cornish Rex, lack guard hairs and may need their ears cleaned occasionally; that should also be handled by a vet.
