That depends on the agreement. In some cases, particularly public defense work, they get a fixed fee based on what they do. There is a specific price for being in court with a defendant and another for visiting them in jail. Most attorneys want a specific hourly rate plus costs. And in contingency cases the money only comes if they win the case.
