It depends what you mean. If you are having a hair test to identify a medical problem, then you will probably need a full cut, as they will need a decent amount of hair (possibly 2 or 3 inches off the length if you have medium to long hair, a bit more if you have very long hair, and maybe an inch or so if you have short hair) However if you are having a test such as a DNA test, then they will just cut a handful out here and there, but you won't notice it unless you have very thin hair.
yes it is cut just off the scalp and they will only need between .5 and 1.5 inches they toss anything over that as all test are standardized by sample wieght.
