No, you don't have to use either.

There are various different sanitary products that you can use during your period, as well as tampons and pads there are also similar options such as sponges and period belts, or totally different options such as menstrual cups or softcups. Tampons and pads are the most widely-known options but not the only options, and not the best options.

Many women don't use sanitary products at all, this is known as 'free-bleeding' where women layer clothing and bleed freely, you can also use what is known as 'menstrual release' which is where women learn to recognize when they are about to bleed and use vaginal muscles to control when the blood leaves the body so they can empty-out blood just like using the bathroom.
