A tooth extraction may or may not require a course of antibiotics before the procedure.
If the tooth is not infected (abscessed) for most people an antibiotic is not required. For some people who are at increased risk for contracting Bacterial Endocarditis, a short course of appropriate antibiotic is indicated.
If the tooth is infected, a few days of antibiotic may be appropriate. Sometimes a local anesthetic will not be effective in the presence of infected tissue. This can result in the extraction being extremely painful.
If one has had an artificial joint placed within the past two years, or a history of heart problems, they should make this known to the dentist. They can make the proper choice of antibiotic coverage based on the patient's health history.

It can be done without antibiotics before (had all four of mine pulled without them, and two were infected) but they should give you something to take afterwards to treat the infection. If not, go talk to a doctor or another dentist.
