It is the expansion pack that comes after Wrath of the Lich King. You do not need to purchase the CD/DVD to play as you can download the client which allows you to play as you are downloading (once you download enough of it). Although you do not need a Cataclysm account to play the basic WoW, you do need it to take full advantage of all the Cataclysm features, zones, and races.

If you want to play cataclysm content, then you need to first upgrade your account to Burning Crusade and then upgrade to Wrath of the Lich King, and finally Cataclysm.

Otherwise, the game works just fine and you can play all content up to and including what was released for Wrath of the Lich King. Note that the changes to the old world (Azeroth and Kalimdor) are changes to the base game, so anyone will have the new quests and dungeons from level 1-60, regardless of what expansions they have installed or not.
