Yes! Hot Chocolate is much healthier when based on water than when made with milk! Cocoa contains many healthy compounds called polyphenols that are neutralized by milk proteins catechins. But if you add cocoa powder to water or melt Dark Chocolate in water, you can still reap those benefits of cocoa!

It doesn't really matter. You can also melt chocolate in a dish in the microwave which is even faster. If you prefer to do over a double boiler then shave the chocolate with a potato peeler or put small chunks to make it melt faster.

It is a bad idea. Hot water, straight from the faucet, contains more dissolved minerals (calcium, assorted salts, etc) than cold water does. It's simply because of the temperature...more solids can be dissolved in hot water than in cold. The same minerals that cause the scaling and "hard water" deposits in your bathtub will also clog up your Mr. Coffee. There are coffee machine cleaners (chemicals) that help remove these deposits, but eventually they'll build up so badly that the machine is ruined. Using hot water, rather than cold, will ultimately shorten the life of your coffee maker.
I haven't seen your recipe, so I can't say for sure, but the hot water is to dissolve the powdered chocolate, otherwise it won't ever dissolve.
You melt the chocolate very slowly with low heat. Put water in a pot and bring it to a hot but not boiling temp. fit another pot over the first one so that it touches the water. Put the chocolate in the post and slowly stir. once melted, the chocolate should be cool/warmish to the touch. if its hot, then you did it wrong. At this rate you can put the chocolate on the glass. It will take a very long time for the chocolate to harden, so you can enjoy your drink for at least a couple of hours.
