Ethics is always present in a hospital. Some things you may consider unethical is a DNR, in which if the patient goes into cardiac arrest or a condition like that, doctors cannot under any circumstances revive the patient, and must allow them to die. Another, is that if a patient is infected with the blood of another patient (for whatever reason) who, say, has HIV. The patient, or even nurse or doctor, must not be aware they have been infected. This is technically leaking patient information out, and is forbidden/ illegal. Ethics is a wide spectrum, and these are just a few examples of possibly unethical behavior that is "legal."

In short, Yes. It is unethical for any kind of doctor to date their patient as their is a power differential between the two people. Besides the fact that it is unethical, depending on your location it may be illegal. In Ontario it is against the chiropractic code of conduct to date your patients, or treat someone you are in a relationship with. Besides the ethical dilemma, there are issues with the quality of care that a significant other receives. Whatever the reason, in Ontario a doctor of chiropractic will lose their license if they are caught dating a patient.
No. The surgical procedure should be what the surgeon books it as. Meaning the surgeon plans on using instrumentation or allograft whether the patient has insurance or not. The only logical reason the rep is looking to see if the patient has insurance is exactly why you think he/she is: suggesting or implying the patient needs more of his/her product. Unethical. They should not even be looking in the chart.
When someone asks you if you are the doctor or the patient, it usually means that you are the patient but you are trying to diagnosis your symptoms as if you were the doctor.
