Those words do not mean the same thing, even though they do have a connection. A hesitation is a pause, someone is not doing something immediately (it could be anything, such as answering a question). We may infer that the reason for the pause is that the person is taking time to think about what he or she is doing, therefore, considering (which is synonymous with thinking). But the thought is not the hesitation, even if it is the reason for the hesitation. The two are not identical.
