Not necessarily- it all depends upon how well maintained the engine has been, and also whether it's the vehicles' original engine or not. A high-mileage engine that has been well looked after and cared for, will be in a better running condition than a low-mileage one that hasn't. Also, if the vehicle has had a replacement engine fitted, then mileage will be irrelevant anyway as far as the engine's condition is concerned.
And what do you mean by "lousy"? Do you make these sorts of comments to your grandparents?

Assuming you mean fuel mileage, we need to know the year, the engine and if it 2 wheel or all wheel drive.

The two most common answers I think are change your oil and if the engine is older with high mileage then the bearings are worn.
i own a 2002 f150 and it has 196,000 miles and still runs great. Over 150,000 miles is considered high mileage on any vehicle. That does not necessarily mean there is something wrong with it.
That just means you get a bad mark on something
For the most part, all the benefits of synthetic engine oils do apply to higher mileage car engines. If, however, the old high mileage engine is very worn (and I mean worn well beyond acceptable limits for normal operation) of if ignition or fuel system malfunctions exist resulting in contamination of the oil, then any "extended drain" benefits from synthetic oil do not apply. In such case it may be more sensible to use the least expensive oil available which meets the engine manufacturer's specifications.
