No it doesn't. It actually feels really good to the cow because all that milk that is making her udder so big and swollen is being released by the milk machine or by her suckling calf. If it would hurt cows to be milked they wouldn't be so eager to get to the milk parlor when their udders are full near to the point of bursting. It's the same with other mammalian mothers: if it hurt to have their newborn offspring to suckle them, then those offspring would die, and we wouldn't have the mammals around on this earth as we know it.
