Healthcare workers are Humans first. We all make mistakes, and we shouldn't be judged for it. We take necessary steps to avoid errors. But , it still happens especially under poor working conditions (short staff, double shifts, no break etc...)
You don't have to reduce the steps.
They reduce training time, the loss of unwritten information, the commission of errors, the omission of essential steps or processes, and the time required for completion of tasks.

Only the facilities that have JCAHO accreditation are going to the "do not use" list. Small hospitals and clinics that are not accredited do not have anyone to prevent them from using dangerous abbreviations. We should have national standards that all medical personnel must follow when documenting and using abbreviations. There should be sanctions placed against those who do not follow the standards.
