A torte is traditionally a multi-layered cake with many layers of filling, and very thin, soft layers of sponge cake. It's very rich, and usually contains cream and ground nuts. Today, torte often refers to any cake that has more than two alternating layers of filling and cake. So, there isn't any hard and fast rule as far as what it should look like.
If the filling is still present, it should be visible to the naked eye, or at least visible on a dental x-ray.
If it is done correctly you should not get a filling. If there is infection in the cavity and it is not removed then there is a good chance you'll have an absess. You may have to take antibiotics the week prior to getting the filling. It just depends on your situation.
