Yes, this has happened. One example is Lincoln in 1864 who replaced Hamlin with Johnson. Jefferson's first VP, Aaron Burr was virtually a fugitive by 1804 and so was replaced. Franklin Roosevelt got a new running in 1940 for his third term. U.S. Grant's first VP , Colfax, was dropped from the ticket for 1872 after charges of bribery were made against him.

There have been several sitting US Presidents who had the opportunity to run for another term of office but chose not to. The most recent one was Lyndon B. Johnson. He declined to run for office in the 1968 presidential elections. Among others were Harry S. Truman and James Buchanan.
