If you mean that it makes a hissing sound WHEN you breathe, then the yawning mechanism in that ear is probably forced open. Try to yawn hard and swallow often (not meant to sound sus)
However, if it's with every beat of your heart, that's just turbulence in your carotids and/or jugulars, nothing to worry about.
