Most kings inherited their power.

The first kings of medieval countries were nearly all tribal chieftains who set those kingdoms up.

Some kings, like Richard III, murdered the rightful heir, and this was especially likely to happen when the heir was a child and the next in line was the regent.

There were coups, such as the one conducted, with the assent of the pope, by Pepin the Short, who previously had been mayor of the palace.

Sometimes a king was given control as a result of a civil war, when a candidate acceptable to both sides could bring the war to an end. Henry VII of England is an example of this.

Some people managed to set themselves up as kings by winning wars or rebellions. An example of the later was Ivaylo of Bulgaria, called Ivaylo the Cabbage, a swineherd who set himself up as a king in a peasant revolt. He was fairly quickly marginalized and assassinated, but the revolt had succeeded in making him king for a short time.
