Most reputable doctors that are the least bit involved with this require that you follow the WPATH http://wpath.org/ Standards of care which say that you need to see a therapist first. So, your first step would be to find a gender therapist in your area. After you have met and discussed your feelings and experiences with a therapist, and they can determine you aren't really psychotically crazy and understand what you are getting involved with (which usually takes between two and 32 sessions) they will give you a letter of approval which you could then take to an endocronlogist and become Hormone therapy.
