This is a long process. Alcohol intake affects the liver metabolism. Now, the liver is an important organ required for the transport and breakdown of fats and lipids. When there is liver damage due to prolonged alcohol intake, the fatty acids cannot be broken down in the liver.
These bit of fatty acids and glycerol are seen in the circulation since they have nowhere else to go. As long as these bits of debris remain in the large vessels, there is no problem, but when they reach the coronary circulation (the group of blood vessels that supply the heart), they are too big to pass through, and occlude the artery.
This sudden loss of oxygen supply to the heart leads to death of the cells in the myocardium, felt as the pain of angina.
