Mainly by shorting out, letting the + and - wires or connectors touch. Even for a split second this can do a world of damage. Some of the larger amps have fuses in the speaker line to help avoid this kind of damage. The next worst is overheating, a cooling fan is not uncommon to find on more powerful amps. I put outboard fans on all of my equipment that generates much heat.
