The mildew smell is probably from your carpet not drying. Shampoo the carpet again using a product that removes mildew, making sure that you remove as much woisture as you can.
Spray Lysol into the fresh air vent near the bottom of your windshield. This will kill the mildew.

One place to check is the A/C drip pan and the A/C ducts. If in A/C Ducts, need to clean or replace them. Also, look on line for mold and mildew remediation techniques.
