To tell if a bone is broken the best way is to get an x-ray done. Depending on which bone and how it might have been broken the x-ray should show what's going on with the bone. Beyond that, there are a few other ways to tell, if you push on it does it move? Do you feel a crack or a grinding?
You can tell somewhat by the stage of healing on an Xray or MRI. A brand new fracture shows no signs of healing. A fracture that is about 2-3 weeks old may show some new bone deposits in the fracture line. As healing takes place, a bony "callous" forms around the fracture, which looks like a cocoon surrounding the fracture. As the fracture site ages, the callous shrinks somewhat but may not disappear completely. Pain can also be used to assess the time a fracture has been present. Fractures cause acute pain (sudden onset, usually severe) and the pain lessens as the fracture heals and the bone fragments become stable and do not move with movement of the extremity, rib cage, etc. Many factors affect healing time, but most fractures heal within 8-12 weeks.

I don't know the specific definition but a remote fracture is one that is not fresh. As in an old fracture. There is probably a rule that it is >6 month or >1 year or something.
