You would have to suspect you are pregnant and do a pregnancy test. My cousin had an IUD fitted after her first child and breast fed him for nearly a year, thinking this was why she had not had a period. In fact she had conceived when he was 7 months old. The baby was born and the IUD was embedded in the placenta.
