A sick Goldfish will be dull looking, have clamped fins, maybe white stringy feces, it will be lethargic too, maybe sitting motionless on the bottom. Things like gulping air at the surface, brown/black spots that weren't there before, or blood streaks in the fins are also bad. Any abnormal bloating (to the point the scales are sticking out) or dramatic weight loss also means the fish is sick.
