Usually by visual inspection. Look for worn cogs and fraying of the belt.
The best way is to replace it on a regular basis, IE every 100,000 miles. If it breaks while the engine is running it can produce severe engine damage and bent valves.

∙ 9y agoIn most instances you absolutely cannot tell by looking if a cam belt needs replacing. It may look good and be ready to break. That is why you replace it at a set mileage.
