Usually by visual inspection. Look for worn cogs and fraying of the belt.

The best way is to replace it on a regular basis, IE every 100,000 miles. If it breaks while the engine is running it can produce severe engine damage and bent valves.

In most instances you absolutely cannot tell by looking if a cam belt needs replacing. It may look good and be ready to break. That is why you replace it at a set mileage.
