Are a special flange and washer required in order to install a wallmount toilet in a basement to take advantage of a nearby drain pipe?

You don't have to have one, it just makes it a little easier. As long as the drain is flush with the floor, you can drill into the concrete and us anchors to hold the toilet down. The down side is that they tend to corrode over time and become stuck in the concrete. If you ever have to remove the toilet, they can break and that makes putting the toilet back harder. You can just bolt a mounting ring to the floor and use regular toilet bolts to hold the toilet. These can be easily replaced if needed.

When installing a shower drain in a concrete floor how do you determine the height of the drain which will protrude through the concrete?

First determine what you are going to install as the shower unit. If there is to be no shower unit and it will just drain through the concrete floor, then it will be flush with the floor. If you are installing a unit, then you need to look at the unit to determine the distance obove the concrete to place the drain.

Toilet wont work on floor drain?

Probably not. There is no vent for that drain, it did not need one for a floor drain. A toilet is trying to push against the air pressure in the drain.Normally a decent plumber will install a floor drain with in 15 feet of a vented line or will vent the FD in case irt exceeds the 15 ft ruleA Floor drain can be as small as 2" diameter and thus it is undersized for a toilet and a floor drain alread has a trap and a toilet would thus be drouble trapped

A toilet has to be vented otherwise you are trying to force liquid against the air in the drain and it is pulling against a vacuum. It will never flush right.
