When people starting concentrating their populations in cities, it increased the number of people exposed at one time to the plague. Also, many individuals living in the cities were exposed to raw sewage, excessive air pollution and inadequate housing; they also suffered from a lack of adequate nutrition and little access to clean water. Because they were physiologically stressed with chronic afflictions, the plague killed many more people than may have died otherwise.

Finally, as people saw the start of the plague in the cities, they fled to the countryside to "get away from the foul miasmas". In doing so, they carried the rat fleas with their bacterial infections to the countryside and to other cities, spreading the plague across the continent.
