Wisconsin was the last state to gain statehood in the old northwest territories. Wisconsin gained statehood on May 29, 1848.
I guess none have been denied. While some territories have significantly delayed petitioning for statehood, including Alaska (92 years) and Oklahoma (104 years), no valid petition for statehood has ever been denied by the U.S. Congress.

Originally the first 13 colonies were land grants from the king or queen of England. New states were added when the territories had enough population to apply for statehood and the Senate and President agreed.

I don't think that any of the current unincorporated U. S. territories have requested statehood to date. However, on November 6, 2012, Puerto Rico voted on whether their status should be independence, free association, or statehood. 61.15% voted for statehood.
