Male and female doctors wore white smocks over their street clothes. The nurses wore the starched caps. Since the 1960's (the 'let it all hang out' generation) came into play doctors are often in suits, or even some are in casual clothing with the exception of being properly 'suited up with smocks. face masks and rubber gloves' during surgery.
