open the panel that is located on the back of the headlight by removing the screws. Pull the panel up to show the back of the headlight. Grab the back of the light bulb socket inside the panel and turn the socket counter-clockwise until you feel the socket come loose, then pull it out of the headlight. To release the bulb from the socket, push the light bulb in and turn the bulb until you feel it come loose. Then remove the old bulb from the socket and replace it using a clean dry cloth to hold the new light bulb. Install the new bulb into the electrical socket and reinsert the light bulb socket into the back of the Lexus LS400 headlight. Lastly, reattach the access panel with screws. Verify the headlight is working by turning on the headlight switch and checking that the bulb is operating properly. Once it is verified, turn the light switch off.
just replace it. not repairable. if the cartridge is messed up, then you can get a new cartridge.

I believe it is under the intake manifold.
As far as I know, you have to replace the cluster.
