How do anorexics get so thin if starving make you hold on to fat?

Anorexics do not eat anything or very little for long periods of time, so eventually the body will burn up all of its fat storage, which is the preferred energy source. After burning up all the fat, it will turn to eating up muscle because protein is a backup source of energy. So if no new energy (a.k.a food) is being consumed then the body is not gaining anything new to work off.

They are different definitions of 'thin'. An anorexic person is thin in an extremely unhealthy way; they lose just as much muscle as fat. A normal 'thin' person just has very little body fat, but the usual amount of other tissue. This makes them far healthier. Anorexics don't completely starve themselves because they'd die. They consume enough calories to survive (usually 300-400 daily, maybe less, like an apple and diet Coke for example) but not enough to gain weight, so they lose it instead. Starving does work but is a terrible idea.

Starving does NOT make you hold onto fat. A starvation diet will use up your body's store of fat, known as adipose tissue. Once the stores are exhausted, the body starts metabolizing muscle tissue -- a very serious, unhealthy condition. When your body goes into "starvation mode," your metabolism slows down in an attempt to conserve energy and your body's fuel stores. This is erroneously thought of as "holding onto fat." But that is like saying your car holds onto gasoline when driving slowly. Clearly, that is preposterous. You may get better mileage at 55 mph than 90, but you're still burning gas, albeit more slowly.
