We had a plumber in for this problem. We have a tile shower and he said the hair dye/smell is in the grout, and to get Clorox bleach gel and apply it to all the grout - not just the floor but the walls, also. Our smell is not there until we start running the shower, which he said fits his theory. Good luck!
Yes...any concrete grout will work.....you're better off regrouting though because chances are that the grout is sealed and the stain will only last one or two seasons.
if you grout has dried different colours then you might have used more then 1 package of grout yet the same colour just different dye lot numbers so if you ever need to use more then one package mix all the grout together then mix it up with water or if it is small parts in different areas you could have the thin set in the grout lines so your seeing that

if your grout line is less than 1/4" you use smooth grout, above that you use sanded grout.
The old grout can be cut out. The mist efficient way is with an oscillating tool with a grout blade. After the old grout is removed then new grout can be installed. This is a time consuming project.

You should use the grout that will work best for the biggest grout space that you need. So for a grout width like you described, 1/16 to 1/2, you should use sanded grout. Unsanded grout will look terrible in a 1/2" grout joint, but sanded grout will look great in both. Use sanded.
