No, seriously. The parasite that causes the disease lives in mayflies, which then swarm around the lights in barns and die, falling into the feed. The horses eat them (and the fluke) and voila- Potomac fever.

The biggest key for prevention is to turn off all unnecessary lights in the barn at night, if you're in one of the areas affected.
