You file the completed forms at the courthouse and attend a hearing. Some states require that you publish a notice in the local newspaper too. It is easier when both parents agree to the child's name change.
Legally changing one's name or the name of one's child is relatively simple and inexpensive. The matter may become a bit more complicated in cases of minors where one of the parents objects to the action. Generally a petition is filed with the probate court to have the child's name amended to that of the mother, father or a new spouse in cases of adoption. The judge sets a hearing and asks a few questions and if there are not parties who can offer acceptable evidence/argument as to why it should not be allowed the petition will be granted. The parent may then contact the record of vital statistics for the state in which the child was born to have the birth cerificate amended. The changing of a child's surname does not affect the responsibility of a parent to pay child support nor have visitation, custodial rights, etc. those are unrelated issues.
you could probaly make your last name his middle name and your husbands last name his last name

Assuming your child NOW has a different last name than your current last name, contact an attorney to petition the court for a name change.
Changing a minor child's name is more difficult than changing your name as an adult…One parent may file the petition and must give notice to the other parent who will have an opportunity to contest the change. A parent has an equal right to let the child have his or her last name, but when there is a dispute, the deciding factor is the child's best interest. The court will decide whether it is better for the child to have her or his mother's or father's surname.
Well if the person you are marrying is not the other biological parent then you would need them to sign rights and have the person you are marrying adopt the child... I just went through this... good luck to you

It is not legally required to change your surname once you have been adopted. It is usually up to the adoptee if they want to change their surname. At least, that was the case with me, I was adopted at the age of 7 and my adoptive parents asked me if I wanted to keep my name or change it. I, of course, changed it.
