I created an extension recently for Google Chrome to do exactly this. It converts the URL you are currently visiting to a QR code and you can scan it with any QR code reader on your phone. I created this extension because I found it annoying when I found a recipe of something on my PC that I needed to take to the kitchen to do the cooking. This solves the problem :)

You could copy the URL from the address bar.
