Pathogens spread from person to person in numerous ways. The influenza virus is transmitted through the air, usually via sneezing and coughing. Escherichia coli is transmitted via water, food, and blood. There are many more pathogens that can be transmitted by saliva, etc.
Human pathogens can be spread from person to person in several ways. Some of the more common means are: skin-to-skin contact - this would occur when a person has a pathogen on their hand, touches something, transferring it to that object, and someone else then touches that object, or simply shaking hands with someone who has the pathogen on their hand - this person would then need to have contact with an open part of the body i.e. eyes, nose, mouth or an open wound, in order for that pathogen to be transferred into their body; airborne - sneezing, coughing, breathing too close to someone and the other person inhales the pathogen; blood-to-blood contact - open wound comes in contact with an open wound, or sharing needles with someone taking illegal drugs; or sexual contact - pathogens, in this case STD-causing pathogens, can be spread through sexual contact of the genitalia via unprotected sex (any type).
Through coughing, touching door handles, sneezing, and lots of other ways that happen anywhere else.
Since it's a hospital, you'd think it'd be really clean and wouldn't have as many germs. But since it's always getting cleaned to prevent the spread of illnesses, the microorganisms evolve and are no longer able to be killed by cleaning products. Those microorganisms multiply, and people get sick.
These are few ways by which the pathogens spread. One is by direct contact. Few viruses spread by direct mouth to mouth contact. Few pathogens spread by sexual contact. Like the pathogens that causes HIV, syphilis, gonorrhea etc. Leprosy and scabies can spread by this way. Few pathogens spread through vectors. Diseases like malaria, dengue, filariasis, scrub typhus etc spread via vectors. Few diseases are water born. They contaminate the water. When this water is ingested by another person, that person gets the disease. Cholera is well known to spread via infected water. Infective hepatitis or hepatitis A can spread this way. Some diseases spread by fecal-oral route. Typhoid, hepatitis A, cholera are the examples of these diseases. The pathogen is excreted in the feces. These feces contaminate the water. The person may not wash his hands with soap and water. He may handle the food and contaminate the food this way. Some diseases are transmitted via air. Flu and common cold viruses are transmitted via air. When you sneeze, millions of small droplets may be released in the air. They will contain the pathogen present in your nasal secretion. These pathogens are inhaled by another person. That another person may get the disease. Some pathogens are spread when you cough. For example pulmonary tuberculosis is spread by this way.
Pathogens can be spread through different modes of transmission. Among these the most common is through inhalation, droplets, direct contact, blood transfusion, and sexual contact.

An airborn disease is one for which the pathogens are spread through the air.
Pathogens can spread though direct contact by animal bites or exchange of bodily fluids. They're spread through indirect contact by contact with nonliving surfaces, air, dirty food, and vectors.
Substances present in the blood that can cause infection or disease. Hepatitis B and hepatitis C viruses are bloodborne pathogens since they are spread through blood and can cause a liver infection.
