Possibility 1) They hang a bottle in a pear tree with the fruit bud inside the bottle. It grows in there - not common anymore. Possibility 2) The Glass Bottle consists of a threaded bottom part. The pear is put into the bottle, then the bottom is "screwed" onto the rest of the bottle which will eventually seal the two parts together as well.

To see the pear in the brandy bottle is mind blowing! The bottle (without the brandy) is placed at the bud of the branch where the fruit will form. The branch while the fruit bud is still very small is put into the bottle opening. When the fruit has fully formed then it is simply broken off from the branch and the pear remains inside.

When pear trees are sprouting and the Pears are growing and are very small, a bottle is attacted to the tree usually with some string and a small branch with a pear on it is placed inside the bottle (while the pear is still attached to the tree). When the pear grows larger and is fully ripe the bottle is detached from the tree and pulled downward so that the pear remains in the bottle with no branches.
