If you have the policy number and the name of the company, you should call the insurance company. If you do not have either but payments are being drafted from an account, check the statement of that account for information on the company then call them with your social security number. I think most states will require that a copy be conveyed within 10 to 30 days depending on your state of residence. Most companies will send a policy holders copy promptly on any request of their insured. Many companies now offer instant online policy access to insureds as well.
As part of your policy, there is a "Policy Schedule" section where all your benefits are explained. If you don't have a copy of your policy, your agent or company can provide a copy.
