"The Reverends (his first name) and (her first name) (their last name)" is the proper way to address an envelope. Dear "Reverends (last name)" would be the opening to a letter. If introducing the couple to another person, "These are the reverends (so and so)" would be appropriate.

It may seem odd, but etiquette would be: "The Reverend Mr and The Reverend Mrs. (Surname)."

This was not a problem until recent times, since the place of leadership in the church was always considered to be a man called by God. The reasoning still is prevalent in many church circles and some denominations are split over the appropriateness of having women as pastors. This stems from the instructions (1 Timothy 3:2; Titus 1:6) for qualifications of pastors, among other requirements, he is to be "the husband of one wife," never being stated the other way as a wife with one husband, nor indicated anywhere in the days of the New Testament church.
