It depends on what sort of toys it is. If it is soft toys you can use little cloth pegs or a baby spoon. You attach the toy to the spoon (use a rbbon to do so) and then stick the spoon in one of the diapers.
If it is a hard toy like a car you might like to try to fix it with several baby spoon you stick upside down in your cake and then squeeze it in there. You can also try to just place the toy on the cake and then wrap the cake with gift foil wrap so the toy is being hold by the wrap and can't fall down.
